Home Care Manager Role

Bolton Clarke is a leading provider of healthcare and aged care services, with over 200 years of experience.

We are seeking a skilled professional to manage the clinical and ongoing care management for recipients eligible for Home Care Packages funded by the Commonwealth government.

Key Responsibilities:

  • Provide comprehensive client assessments and develop personalized care plans to support clients at home.
  • Lead governance and quality management frameworks within HomeCare services to ensure care standards are met.
  • Deliver direct and indirect clinical care to care recipients within scope of practice and maintain their clinical needs.
  • Contribute to program development, including participating in procedure development, planning, and reviewing care recipient services.

Requirements:

  • Mandatory Nursing bachelors degree qualification and registration.
  • Highly developed assessment skills and creative problem-solving abilities for complex care situations.
  • Knowledge and experience in community-based care and support services.
  • Dynamic approach to providing high-quality care.
