Family Nurse Practitioner - PRN

We are seeking a compassionate and skilled Family Nurse Practitioner to provide exceptional patient care in a collaborative team environment.

As a key member of our interdisciplinary treatment team, the Family Nurse Practitioner will work closely with patients to assist in chronic disease management through individual interventions, clinic support, and ongoing education.

The ideal candidate will have experience providing primary care and diagnostic services in a clinical setting, as well as a strong desire to promote health, disease management, and prevention.

Responsibilities:

  • Provide high-quality, comprehensive primary care to patients ages 2+
  • Work collaboratively with the clinic staff to develop and implement individualized care plans
  • Prescribe appropriate medications, treatments, and health regimens to treat patient health issues
  • Conduct regular health assessments and screenings to identify potential health risks
  • Develop and maintain accurate and up-to-date patient records

Preferred Qualifications:

  • Occupational Health and Worker's Compensation experience
  • Experience working in a similar healthcare setting
