The role involves delivering comprehensive services across home care clients in the Mornington Peninsula area.
You will be responsible for conducting clinical assessments, developing care plans, and providing nursing services to clients. This is a dynamic opportunity to work holistically with other healthcare professionals and collaborate with Case Managers and NDIS Support Coordinators.
Key Responsibilities:- Conduct thorough clinical assessments to identify client needs
- Develop appropriate care plans that meet client goals
- Provide a range of nursing services including wound management and medication administration
- Work collaboratively with multidisciplinary teams to achieve optimal client outcomes
- Communicate effectively with clients, families, and healthcare professionals
- Unrestricted Registration with AHPRA as a Registered Nurse
- Sound clinical knowledge and skills with experience working with clients who have chronic and complex health needs
- Excellent verbal and written communication skills
- National Police Check Clearance
