About the PositionWe are seeking an experienced Case Manager to join our multidisciplinary team in the Dubbo Brain Injury Rehabilitation Program (BIRP).
The BIRP provides a community outreach case management rehabilitation service to adults with acquired brain injuries in the Western NSW Local Health District.
In this key role, you will be responsible for ensuring delivery of contemporary brain injury rehabilitation in accordance with the principles and guidelines of the NSW BIRP.
This is an exciting opportunity to work collaboratively with a dynamic team of Senior Clinicians, including three clinicians in Brain Injury and a Rural Spinal Cord Injury Coordinator.

- Travel within the Health District to deliver services
- Work collaboratively with multidisciplinary team members
- Provide support and guidance to clients with acquired brain injuries
You will:
- Deliver high-quality case management services to clients with acquired brain injuries
- Develop and implement individualized rehabilitation plans
- Collaborate with other healthcare professionals to ensure comprehensive care
- Maintain accurate records and reports
