Job Title: Care Team Coordinator

About The Role

This is an exciting opportunity for a dedicated and compassionate individual to take on a key role in the delivery of high-quality care to our residents. As a Care Team Coordinator, you will play a vital part in ensuring that the needs, wishes, and goals of each resident are met within a supportive and respectful environment.

Key Responsibilities:
  • Provide social and emotional support to all residents, ensuring that care provision is holistic and person-centered.
  • Support the coordination of care needs of residents across the facility, including medication management, nutrition and hydration, and pressure care management.
  • Lead and supervise care on shifts, within designated wings or areas.
  • Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and policies.
About You

We are seeking an individual who possesses the following skills and qualifications:

  • Diploma of Enrolled Nursing (with AHPRA Registration to administer medication)
  • High level of commitment, integrity, honesty, and trustworthiness
  • Effective listening and communication skills, both verbally and in writing
  • Ability to understand and use information technology and programs, and to learn new skills as required
  • Current National Police Check
  • Working knowledge of WH&S and Infection Control
