Job Title: Home Care Specialist

This role involves working with older Australians to ensure they receive the right care and support in their own homes.

The ideal candidate will be a skilled people leader with experience in community care, aged care standards, funding, and compliance. They will be responsible for assessing client needs, developing care plans, coordinating services, and advocating for clients.

  • Assess Client Needs: Conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring care plans with clients and families.
  • Care Planning: Develop, review, and update person-centered care plans aligned with client goals and Support at Home funding.
  • Coordinate Services: Organize in-home supports and liaise with external providers to ensure timely, quality care.
  • Advocate and Educate: Support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
  • Budget Oversight: Monitor individual budgets and guide clients on effective use of funds.
  • Relationship Building: Build strong connections with clients, carers, and providers, supporting smooth transitions and ongoing care.
Requirements:
  • Cert III in Individual Support (Ageing) or equivalent
  • Experience leading a team in aged or community care
  • Working knowledge of aged care standards, funding, and compliance
  • Current First Aid and CPR certification
  • A reliable, comprehensively insured vehicle and a valid driver's licence
  • Strong communication, organisation and problem-solving skills
