Job Description
- The Support Coordinator for Complex Needs plays a pivotal role in providing consultation and coordination services to clients with intricate support requirements.
- This position acts as a single point of contact for service providers, clinicians, and individuals seeking information about complex needs service responses and interventions.
Key Responsibilities
- Provide consultation and coordination services, facilitating service planning for clients with complex support needs, including MACNI and SfHRT clients.
- Foster relationships between departmental program areas and external services, offering capacity building regarding support provision for clients with complex needs.
- Promote integrated responses and practice excellence by partnering with service providers.
- Identify systems gaps, risk factors, and training opportunities to improve service systems for clients with complex needs.
- Support the Area Client Support and Housing unit, representing the program in various environments.
Accountabilities
- Offer authoritative and expert advice and recommendations with a specific program or area-based focus, aligning with organisational goals and objectives.
- In collaboration with the relevant Manager, manage the allocation of resources within the work area to achieve service delivery goals.
- Interpret and apply business plans and policies in own area of responsibility, providing advice to others on design and implementation.
