About This Role

We are seeking a committed and compassionate professional to join our team in Toowoomba, providing case management support within both community and residential settings. This role plays a key part in assisting individuals to achieve housing stability, greater social inclusion, and a sense of self-mastery that enables long-term tenancy and improved quality of life.

This is a challenging yet highly rewarding role, where you will work closely with clients to develop tailored plans that meet their unique needs and goals. You will be responsible for conducting thorough assessments of needs, risks, and vulnerabilities, and implementing and monitoring case plans in partnership with clients.

The successful candidate will have strong communication and interpersonal skills, with the ability to build effective relationships with clients, colleagues, and other stakeholders. They will also possess a high level of empathy and understanding, with the ability to work with clients who may be experiencing complex and sensitive issues.

In this role, you will:
  • Ensure service delivery aligns with the National Homelessness Stream Model of Care, supporting a consistent, high-quality approach to homelessness services across the country.
  • Building effective, supportive relationships with clients that promote self-determination and active participation
  • Delivering tailored case management in line with The Salvation Army's models and best practices
  • Conducting thorough assessments of needs, risks, and vulnerabilities
  • Providing ongoing evaluation of client needs throughout the support journey—from intake through to exit
  • Collaboratively developing individualised case plans that include practical interventions and strategies to overcome barriers
  • Implementing and monitoring case plans in partnership with clients
