We are seeking a dedicated and compassionate Case Manager to join our team in delivering tailored care to enhance the quality of life for older people.

Job Description:

Key Responsibilities:

  • Provide in-home assessments, referrals, and care plan reviews & goal planning for consumers
  • Manage consumer budgets across different level packages
  • Exhibit excellent time management with client caseloads
  • Respond and communicate effectively with clients regarding changing care needs
  • Maintain strong relationships with internal and external stakeholders
  • Demonstrate empathy and understanding towards the needs of older people

Requirements for Success:

  • Solid case management experience
  • Regular travel around the Bateman's Bay area
  • Knowledge of and enthusiasm for community aged care services and consumer-directed care environments
  • Diploma-level or above qualification in community, aged, health, or another discipline desirable
  • Open driver's license
  • Access to a reliable, registered motor vehicle with full comprehensive insurance
  • Strong computer skills
  • COVID vaccinations including booster
  • A clear national police check upon role offer
