Community Care Nurse

About the Role- As a Community Care Nurse, you will provide person-centred care to your clients in the community, ensuring a safe environment and applying your clinical skills to improve quality of life through activities that include:
- Assess, plan, and implement client care within your scope of practice
- Consider physical, social, and psychological requirements
- Create and update care plans to reflect developing needs
- Ensure clinical quality compliance standards are met
- Work collaboratively with other multidisciplinary team members
You will spend most of your day on the road, completing scheduled community visits.
Role Requirements- Full, unrestricted registration with AHPRA and unlimited work entitlements
- General nursing experience post-registration
- Excellent communication and clinical problem-solving skills
- Australian Driver's Licence (Green Provisional minimum) - Essential
- Roadworthy and insured vehicle to use for work - Essential
- Confidence in managing time effectively, including ability to work autonomously
