About the Disability Support Role

Support individuals living with a disability to achieve their goals and maintain their independence in a community-based setting. This role involves empowering clients through strengths-based service delivery, providing support for goal planning, and promoting health and wellbeing.

Your Key Responsibilities

  • Provide individualised support to clients living with a disability to access the community, live in their chosen residence, and maintain their quality of life.
  • Collaborate with case managers to develop and implement future goal plans and support plans.
  • Maintain client records and contribute to continuous quality improvements.
  • Communicate effectively with stakeholders and clients.

About You

  • You will possess effective interpersonal and communication skills, accurate decision-making skills, and the ability to work in a fast-paced environment.
  • A relevant qualification or experience in disability support is required, along with a current Australian drivers' license and compliance with the Disability Services Act 2006 screening requirements.
