Job Overview

Delivering High-Quality Patient Care in Community Settings is a vital role where you play a pivotal part. As part of our Hospital in the Home service, you will work closely with each client, their carer, General Practitioner and other relevant healthcare providers to develop a plan that meets the client's care needs.

As a skilled professional, you will provide nursing care to people in their community setting, ensuring they receive the best possible care and support.

Key responsibilities include:

  • Working collaboratively with clients, carers, General Practitioners and other healthcare professionals to deliver comprehensive care plans.
  • Conducting thorough assessments to identify individual care needs and developing personalized care plans.
  • Providing high-quality nursing care and supporting clients to maintain their independence and well-being.
  • Maintaining accurate records and reporting any concerns or issues to relevant authorities.
