Role Summary:
The Elder Care Connector will be responsible for building relationships with local Elders, older Aboriginal and Torres Strait Islander people, and their families to raise awareness of their aged care entitlements. This role will work closely with the Elder Care Support Coordinator to identify service gaps and advocate for clients as needed.
Key Responsibilities:
- Establish and maintain relationships with local Elders and their families to understand their aged care needs and preferences.
- Work with the Elder Care Support Coordinator to identify service gaps and develop strategies to address them.
- Provide information and support to clients to help them navigate the aged care system.
- Advocate for clients to ensure they receive the care and support they need.
- Collect and analyze data to inform service development and improvement.
Requirements:
- Be a person of Aboriginal descent and self-identify as an Aboriginal person.
- Have experience working in a health or care sector role with older Aboriginal and Torres Strait Islander people.
- Have knowledge of the aged care landscape or be willing to learn.
- Have strong record management, data collection, and reporting skills.
