The role of a Senior Social Worker involves working with patients who have life limiting illnesses across various care settings.
This dedicated multidisciplinary team supports clients, their families and carers. The team includes Specialists Nurses, Occupational Therapists, Dietitians, Medical Officers and is supported by Clinical Nurse Consultants, Nurse Practitioners and Educators in Palliative Care.
We work closely with other care providers and services to support clients, their families and carers. Our consultation service provides care in the community, residential aged care facilities and inpatient hospital settings.
Utilizing telehealth enabled modalities, including face to face and virtual consultations, we aim to promote quality of life and dignity for patients.
- Develop individualized care plans that meet the unique needs of patients.
- Provide emotional support and counseling to patients, their families and carers.
- Collaborate with healthcare professionals to ensure comprehensive care.
As a Senior Social Worker, you will be responsible for providing high-quality patient-centered care, while working collaboratively with the multidisciplinary team.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a dynamic healthcare environment.
