Key Responsibilities- Engage with clients in a professional and approachable manner, demonstrating empathy, consideration, and respect.
- Provide information, coaching, and mentoring to care and other staff to support high-quality and effective service delivery.
- Actively participate in both informal and formal performance management processes, including annual and ad-hoc performance review, improvement, and development.
- Tertiary qualification in social work and eligibility for membership of the Australian Association of Social Work (AASW).
- Demonstrated experience providing social work services to adult clients in rehabilitative environments, including assessing and evaluating their needs and delivering both group and individual support.
- Excellent verbal and written skills in the provision of clear, succinct, and accurate assessments, care, and support plans, and case notes.
- Current Driver's License.
- First aid & CPR certificate.
- Valid NDIS Worker Screening check or be willing to obtain one.
- Experience working as a social worker in the aged care sector.
- Experience delivering services in a Consumer Directed Care environment, Trauma-Informed Practice.
- Access to a private motor vehicle.
