Lead Clinician Social Worker Job Opportunity

This is a part-time position available within the Older Persons Community setting, with an expectation of crossover work with the Older Persons Inpatient unit to ensure continuity of care.

The role involves providing social work services to patients within the Older Persons Mental Health Service. You will work closely with multidisciplinary teams to provide comprehensive patient care and support family sensitive practice in mental health care.

About the Role

This job requires a clinician who has strong communication skills, a passion for delivering exceptional care, and a commitment to working collaboratively with colleagues and stakeholders.

You will have experience working in a healthcare environment, preferably in a social work or clinical capacity, and possess the necessary qualifications and registration requirements to fulfill this role.

Key Responsibilities
  • Provide high-quality social work services to patients within the Older Persons Mental Health Service
  • Work collaboratively with multidisciplinary teams to deliver comprehensive patient care
  • Support family sensitive practice in mental health care
  • Develop and implement individualized treatment plans to meet patient needs
