Job Title: Social Worker Level 3

We are seeking a highly skilled and dedicated Social Worker to join our team.

  • This position is for a full-time Social Worker to provide specialist social work services to patients, clients, consumers and their family/carers.
  • The successful candidate will conduct early psycho-social assessments, care planning, education, advocacy, and liaison with community agencies.
  • The role involves maintaining social work and case management responsibility for multiple cases until discharge.
  • The position functions as part of an inter-disciplinary team providing low-intensity therapy home-based programs to frail elderly clients.

Requirements: To be successful in this role you will need:

  • A Bachelor Degree in Social Work or equivalent.
  • Current registration with AASW.
  • Strong assessment and planning skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
