  • The successful candidate will complete timely and accurate patient assessments, develop and update individualized care plans based on patient needs.
  • Direct patient care and evaluate outcomes, incorporating principles of family-centered care.
  • Monitor and document patients' conditions and the effectiveness of interventions.
  • Evaluate educational needs of patients and their significant others, considering cultural requirements and other special concerns.
  • Deliver teaching and educational materials to patients and/or their significant others as needed.
  • Validate and document patient understanding of teaching.
  • Administer prescribed medications according to established policies and protocols using barcode scanning technology.
  • Implement ordered treatments in accordance with approved policies, protocols, and educational material procedures.
  • Utilize knowledge of one's role and other healthcare professions to assess and address health care needs of patients and populations served.
  • Demonstrate commitment to providing evidence-based practice by adhering to protocols for specific patient populations, reporting needed changes supported by evidence, and participating in evaluating patient outcomes utilizing evidence.
  • Delegate appropriate components of the nursing care plan to other team members.
  • Maintain confidentiality regarding patient information by promoting professional interdisciplinary communication.
  • Respect Patient Rights and adhere to a Patients' First Philosophy.
  • Incorporate CCAD Professional Practice model and patient safety principles into clinical practice.
  • Report errors and near-misses promptly.
