About Our Family Connect and Support Service
We bring together families, support services, and community resources to prevent difficult situations from escalating. As a Case Worker, you will conduct initial assessments of family strengths and needs, determine referral pathways, and follow up with services.
Key Responsibilities:
- Conduct thorough assessments of family strengths and needs.
- Determine suitable referral pathways based on goals.
- Follow up with services to ensure successful engagement.
- Assess and manage child protection risks.
- Liaise with key stakeholders in the community.
- Maintain accurate and timely documentation.
