We are seeking a skilled and dedicated Commonwealth Home Support / DVA / VHC Care Case Manager to join our Home Care Team in Launceston, Tasmania.
As a Commonwealth Home Support / DVA / VHC Care Case Manager, you will have the opportunity to make a real difference in the lives of our consumers by providing day-to-day management of client case loads, care planning, and provision of consumer directed services.
The successful candidate will possess relevant experience in case management, community services, aged care, or equivalent. Knowledge of CHSP and VHC funding is essential. They will also have high-level planning and organisational skills, as well as a desire to create a positive experience for our consumers.
Key Responsibilities:
- Provide day-to-day management of client case loads
- Care planning and provision of consumer directed services
- Making a real difference in the lives of our consumers
Requirements:
- Relevant experience in case management, community services, aged care, or equivalent
- Knowledge of CHSP and VHC funding
- High-level planning and organisational skills
