We are seeking a proactive and energetic individual to join our Shared Living team based in Concord West. As a Support Worker, you will play a vital role in supporting and empowering people with disabilities in their Supported Independent Living (SIL) accommodation.

Key Responsibilities:

  1. Provide support to clients in their group accommodation during active hours, including overnight shifts when required.
  2. Ensure household tasks meet organisational requirements, maintaining a clean, tidy, and sanitized environment.
  3. Assist clients with daily routines, which may include personal care, medication administration, special menu planning, laundry, and leading activities in their homes and out in the community.
  4. Maintain accurate progress notes and incident reporting to meet organisational and NDIS requirements.
