Care Management Role

We are committed to delivering exceptional care and services to our residents. As a Clinical Nurse Manager, you will oversee the delivery of high-quality, person-centered care, working closely with a multidisciplinary team.

Your Key Responsibilities
  • Conduct comprehensive assessments of resident care needs
  • Provide leadership and support to clinical staff to ensure delivery of person-centered care
  • Collaborate with the Residential Manager to identify training needs and performance management of clinical staff
  • Facilitate the admission process for new residents
  • Review resident assessments prior to requests for AN-ACC re-classifications
  • Complete Mandatory Quality Indicator Data
  • Analyze monthly clinical KPI data
  • Investigate clinical incidents
  • Support staff in enabling transition care clients to achieve optimal outcomes
  • Ensure compliance with health, safety, and quality standards
Your Skills and Qualifications
  • AHPRA Registration (Registered Nurse)
  • Leadership experience in aged care, preferably as a clinical nurse manager
  • Comprehensive knowledge of evidence-based clinical practices
  • Demonstrated understanding of continuous improvement, accreditation requirements, AN-ACC funding requirements, and documentation
  • Experience with clinical problem solving and commitment to delivering high-quality, resident-focused care
  • Satisfactory completion of relevant personnel checks
