About the Role
St Vincent's Hospital Melbourne is seeking a highly skilled physiotherapist to join our Hospital in the Home (HITH) team.
As a HITH physiotherapist, you will work with a multidisciplinary team to provide acute, admitted clinical care within a bed substitution and emergency department diversion model.
Our HITH team provides care to patients in their own homes, and we are looking for a physiotherapist who is passionate about delivering high-quality, patient-centered care in a community setting.
Key Responsibilities
- Provide optimal physiotherapy management to patients in their own homes, working effectively with patients, carers, and other healthcare professionals.
- Assess, treat, and evaluate patients with a range of conditions, including cardiorespiratory, orthopaedic, and functional decline.
- Develop and implement individualized treatment plans, ensuring that patients receive the best possible care and achieve their goals.
- Collaborate with the HITH team to provide comprehensive, patient-centered care, including liaison and discharge planning.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient care.
Requirements
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a physiotherapist.
- Relevant clinical experience, ideally in a community setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
- Ability to work independently and as part of a team, with a solutions-focused approach to problem-solving.
