Job Summary:
We are seeking a skilled Nurse Practitioner to join our team as a Patient Care Coordinator. This role involves providing holistic care to patients, assessing their needs, and developing individualized plans to meet those needs.
Key Responsibilities:
- Assess patient needs and develop care plans in collaboration with interdisciplinary teams
- Provide education and support to patients and families on disease management and self-care
- Collaborate with healthcare providers to ensure continuity of care
- Monitor patient outcomes and adjust care plans as needed
- Participate in quality improvement initiatives to enhance patient care
Requirements:
- Diploma in Nursing or Bachelor Degree in Nursing required
- Active registration with the Nurses Association of New Brunswick required
- Minimum two years experience in specialized nursing, preferably in family practice, surgical, palliative care, or oncology
- Valid Class 5 Driver's License and good driving record required
- Clean criminal record check and current CPR Level C certification required
- Excellent communication and interpersonal skills required
- Able to work efficiently as part of a team and possess leadership and coaching skills
