We are seeking a skilled healthcare professional to join our team as a Care Integration Nurse (or Paramedic). As part of our multi-disciplinary team, you will work collaboratively with Medical Officers, Nurse Care Coordinators, and other stakeholders to deliver patient-centred care to suitable patients for MHH.
The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality, safe, and effective health care services.
About This Role
This is an exciting opportunity to work in a dynamic environment, applying your clinical skills to ensure the best possible outcomes for our patients.
You will undertake visits in the community in our patients' homes, delivering nursing care as required by their care plan. Our company vehicle is available for patient visits, starting from our office.
About You
We are looking for a motivated and dedicated individual who is passionate about delivering patient-centred care. If you are a natural communicator, a critical thinker, and a lifelong learner, we encourage you to apply.
Key Requirements:
- Bachelor of Nursing or Bachelor of Paramedicine & current unrestricted AHPRA registration
- Current Basic Life Support certification
- Working with Children Check (WWCC) or willing to obtain
- Clinical experience in the management of acute medical and surgical patients, Intravenous cannulation skills, or the willingness to achieve this competency
- Understanding of National Safety and Quality Health Service Standards
