Anglicare Victoria is seeking a skilled and compassionate Case Manager to join our Residential Care team. As a key member of our team, you will work closely with children and young people residing in our Residential Care facilities, providing support and guidance to help them achieve their goals and overcome challenges.
About the Role:- Work collaboratively with Residential Care staff, DFFH Case Management, and external stakeholders to develop and implement individualized plans for children and young people.
- Provide case management services, including assessment, planning, and coordination of support, to ensure the best possible outcomes for children and young people.
- Develop and maintain relationships with children, young people, and their families to ensure a high level of engagement and participation in the care planning process.
- Contribute to the development and implementation of policies and procedures that promote the safety and well-being of children and young people.
- Tertiary qualifications in social work, psychology, youth work, or a related field.
- Experience in case management, preferably in a Residential Care setting.
- Strong communication and interpersonal skills, with the ability to work effectively with children, young people, and their families.
- Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
