Key Responsibilities- Develop and implement individualized support plans that cater to the unique needs and abilities of each customer
- Collaborate with customers, their families/carers, and other healthcare professionals to achieve individual goals and improve quality of life
- Provide active support, care, and encouragement for individuals with disabilities
- Maintain accurate records and communicate effectively with relevant stakeholders
To be successful in this role, you will need:
- Certificate IV in Disability, Aged Care, Individual Support, Community Service or related qualification, or proven relevant experience
- Knowledge of NDIS Quality and Safeguards Commission Practice Standards and Code of Conduct
- Demonstrated experience providing support to people with disabilities
- Excellent interpersonal skills, with the ability to build strong relationships with customers and colleagues
- Ability to problem-solve and work autonomously, with minimal supervision
