This is a part-time position, with a set roster that allows for a healthy work-life balance. We offer a supportive and inclusive work environment, where you can make a real difference in the lives of others.

Key Responsibilities
  • Develop and implement individualized support plans that cater to the unique needs and abilities of each customer
  • Collaborate with customers, their families/carers, and other healthcare professionals to achieve individual goals and improve quality of life
  • Provide active support, care, and encouragement for individuals with disabilities
  • Maintain accurate records and communicate effectively with relevant stakeholders
Requirements

To be successful in this role, you will need:

  1. Certificate IV in Disability, Aged Care, Individual Support, Community Service or related qualification, or proven relevant experience
  2. Knowledge of NDIS Quality and Safeguards Commission Practice Standards and Code of Conduct
  3. Demonstrated experience providing support to people with disabilities
  4. Excellent interpersonal skills, with the ability to build strong relationships with customers and colleagues
  5. Ability to problem-solve and work autonomously, with minimal supervision
