We are seeking an experienced Case Manager to join our Journey to Social Inclusion (J2SI) program. This innovative program focuses on breaking the cycle of homelessness through intensive, person-centered support. As a Case Manager, you will provide long-term, trauma-informed case management and service coordination, supporting clients to achieve stable housing, independence, and social inclusion.

Key Responsibilities
- Provide intensive case management with a small caseload, supporting clients to secure and maintain stable housing.
- Utilise a trauma-informed approach to help clients develop life skills, improve well-being, and foster social inclusion.
- Work in an outreach capacity, meeting clients in the community.
- Collaborate with local service providers and stakeholders to ensure holistic support.
- Monitor client progress and maintain accurate records in line with program requirements.

Requirements
- Minimum 3-year degree in social work, psychology, or a related field (or equivalent experience).
- Experience working with homelessness, mental health, AOD, or family violence support services.
- Strong understanding of case management, risk assessment, and trauma-informed practice.
- Excellent communication skills and ability to engage with diverse stakeholders.
- Cultural awareness and ability to provide culturally safe support, particularly for First Nations clients.
