Homeless Support Services Manager

At Launch Housing Limited, we are committed to addressing homelessness and its underlying causes. Our Case Manager – Housing Mental Health Pathways Program plays a critical role in supporting people who are at risk of homelessness by providing specialized housing interventions at the time of discharge from hospital.
The successful candidate will work with people experiencing or at risk of homelessness, engaging them during their admission in the acute mental health program and collaborating with hospital staff and community supports to develop an appropriate housing support plan alongside regular discharge planning processes. You will assess clients on the ward or where they are located post-discharge, offer housing assistance, and make referrals to local agencies.
This is a permanent part-time opportunity based in Fitzroy, with 68.4 hours per fortnight. We offer a supportive and inclusive work environment, opportunities for professional development, and a competitive salary package, including superannuation and NFP salary packaging.
