Job Description

We are seeking a skilled Clinical Aged Care Assessor to join our team. As a key member of our multi-disciplinary team, you will play a crucial role in identifying the needs of older adults and connecting them with services that support independent living.

Your Key Responsibilities:

  • Conduct thorough assessments to identify the needs of older adults.
  • Facilitate referrals to essential services that support independent living.
  • Promote the retention and maximization of function and capability in daily living.
  • Work closely with families and carers to ensure their needs are met.
  • Ensure all assessments are conducted in line with Department of Health contractual requirements.

About You:

  • You have at least 1 year of experience in aged care, including client assessments and/or service delivery.
  • You hold tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work).
  • You have current unrestricted General Registration with AHPRA or relevant professional association.
  • You are proficient in Word, Excel, Outlook, and client management systems.
  • You have a current Australian driver's licence, comprehensively insured vehicle, and the ability to travel within your service area.
