">We are seeking a dedicated and compassionate Social Care Coordinator to join our team in Newstead, TAS. In this role, you will be responsible for delivering social, emotional, and recreational support to enhance the quality of life for older people.
You will coordinate lifestyle and social care programs tailored to individual resident needs, fostering a supportive environment through meaningful activities and therapeutic services.
As part of our multidisciplinary team, you will build positive relationships with residents, families, and the community, maintaining accurate documentation and contributing to care planning.
Minimum 2 years' experience in a relevant role
Certificate IV in Leisure and Lifestyle or Diploma in Leisure and Health (or willingness to obtain)
First Aid Certificate
Proficient in computer systems and information management
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Understanding of aged care quality standards and person-centred care principles
National Police Check and appropriate working rights in Australia
Experience in delivering complementary therapies and recreational services
Demonstrated experience in dementia care
Experience working in residential aged care settings
Knowledge of and connections with local community groups and support agencies
