Job Title: Nursing Care Professional

About the Role:

The nursing care professional plays a vital part in our interdisciplinary team, working in the Alternate Level of Care unit at St. Joseph's Healthcare Hamilton. The role involves providing exceptional nursing care to promote health and well-being for our clients.

  • This position requires the ability to assess client needs and provide supportive, preventive, therapeutic, palliative, and rehabilitation means to attain or maintain optimal function.
  • We are looking for a dedicated individual who can model professional values, beliefs, and attributes, and practice in accordance with our mission and values, policies, and procedures, as well as College of Nurses Standards and Guidelines.
Main Responsibilities:

The nursing care professional will be responsible for:

  • Providing nursing care for less complex clients whose outcomes are more predictable and risk for negative outcomes is low.
  • Recognizing changes in acuity and/or complexity in client needs and consulting with a Registered Nurse (RN) in determining next steps in provision of care.
Qualifications:
  • A current registration with the College of Nurses of Ontario is essential.
  • An education in one of the following categories is required:
    • A Practical Nursing Diploma from an accredited Community College program with evidence of IV medication administration as part of the basic educational curriculum.
    • A Practical Nursing Diploma from an accredited Community College program and IV medication administration certificate from an accredited Community College program if not included in basic diploma curriculum.
    • A Practical Nursing Certificate from an accredited Community College program, a medication administration certificate and an IV medication certificate from an accredited Community College program.
  • A current BCLS/HCP certification is mandatory.
  • Demonstrated knowledge, sensitivity, and expertise in the care of seniors and their families.
  • Gentle Persuasive Approach training is preferred.
  • A demonstrated ability to function to the full scope of nursing practice per the College of Nurses of Ontario.
  • A demonstrated ability to communicate effectively (written and verbal) with patients/clients, families, and other members of the healthcare team.
  • A demonstrated ability to set priorities and organize work in response to client/patient needs.
  • Demonstrated knowledge of transfers and lifts.
  • Demonstrated commitment to ongoing learning, professional development, and continuous quality improvement.
  • A basic skill level with computers, MS Word, Internet searches, data entry, and patient information systems.
  • Excellent work performance and attendance record is essential.
