Job Overview:

We are seeking a highly skilled Home Care Support Professional to join our team.

The successful candidate will provide essential administrative support and in-home direct care to our home care services, ensuring the smooth running of our operations.

Main Responsibilities:

  • Conduct assessments at consumers' homes, developing and reviewing individual care plans;
  • Liaise with care coordinators and managers to coordinate consumer care reviews;
  • Refer external service providers and support care staff with consumer care requirements and changes;
  • Maintain detailed records and documentation in accordance with home care package guidelines for consumers;
  • Attend regular meetings and participate in education programs and training.

Requirements:

To be successful in this role, you will have:

  • Proven experience in an administrative support role, preferably within the aged care or home care sector;
  • Strong communication and interpersonal skills, with the ability to liaise effectively with clients, families and the care team;
  • Excellent organisational and time management skills, with the ability to prioritise tasks and meet deadlines;
  • Proficient in using Microsoft Office suite, including Word, Excel and Outlook;
  • Certificate in Aged Care, Community Service or related discipline;
  • A genuine interest in and commitment to supporting older adults and their families;
  • A positive, friendly and helpful attitude.
