Nurse - Community Care Specialist

We are seeking an experienced and compassionate Enrolled or Registered Nurse to join our team at Right at Home Far North Queensland. As a community-focused healthcare provider, we offer a rewarding work environment where you can make a real difference in the lives of our clients.
Key Responsibilities- Provide high-quality nursing care to clients in their homes, including basic care, wound management, and medication administration.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with our Care Manager and other healthcare professionals to ensure seamless care coordination.
- Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed.
- Maintain accurate and detailed records of client interactions and care provided.
- Current registration with AHPRA.
- Relevant Professional Indemnity Insurance.
- Satisfactory Australian National Police check.
- Current or willing to obtain a first aid and CPR certificate.
- Smart mobile phone.
- Driver's licence and reliable vehicle.
