About the Role

We are seeking a highly skilled and experienced Rehabilitation Coordinator to join our team at Brightwater. As a key member of our multidisciplinary team, you will play a critical role in overseeing the day-to-day coordination of our rehabilitation program, ensuring that it aligns with individual client outcomes.

Key Responsibilities
  • Collaborate with the Program Manager and Roster Coordinator to develop rosters based on client needs and budget considerations.
  • Coordinate client referrals, admissions, transfers, and discharges, ensuring that care and rehabilitation practices adhere to relevant codes of ethics, standards, and legislation.
  • Develop and maintain effective communication with clients, families, and colleagues, with an emphasis on conflict resolution and problem-solving.
  • Identify areas for improvement and facilitate quality improvement activities, acting as a change agent within the team.
  • Participate in staff recruitment, mentoring, supervising, and supporting team members, as well as assisting with budget preparation.
  • Collaborate with the multidisciplinary team, internal departments, and external stakeholders to evaluate and improve the rehabilitation service model and foster community-based client interactions.
Requirements
  • Registered with (or eligible for registration) Australia Health Practitioner Regulation Agency (AHPRA), or membership with relevant professional body (where AHPRA registration not applicable) and/or significant relevant rehabilitation training and experience (preferably neurological impairment).
  • Strong commitment to providing safe high-quality care.
  • Working knowledge of the principles of rehabilitation and person-centered care.
  • Evidence of effective performance in leading a team and in the coordination of resources within a service care environment.
  • Evidence of advanced problem-solving skills and a proactive approach to problem prevention.
  • Demonstrated knowledge of human resource management, including experience in staff recruitment and selection, performance management, and training and development.
  • Able to effectively communicate (both written and verbal) in various environments with a wide range of people.
  • Evidence of understanding and use of quality principles within a rehabilitation environment.
  • Demonstrated ability to function as part of an interdisciplinary team and foster effective working relationships.
  • Evidence of effective self-management principles.
  • Working knowledge of Occupational Safety and Health legislation; Aged Care Act and/or Disability Services Commission Standards.
  • Possess a current Western Australian 'C' Class Driver's Licence.
  • Satisfactory National Police Certificate obtained within the last 12 months.
  • Hold a valid and satisfactory NDIS Worker Screening Check.
