About the Role:

You'll have the opportunity to work in a dynamic and supportive environment, connecting with people from diverse backgrounds and circumstances. Your role will involve:

  • Supporting individuals with a disability to develop personalized plans that align with their strengths, skills, and interests
  • Guiding and assisting people with a disability to build capacity and explore options for self-direction, self-management, and self-advocacy
  • Providing information and support to help individuals and their families access mainstream services and participate in their communities
  • Building and maintaining positive partnerships within the community to refer participants to a range of services and supports
  • Developing strong relationships with local service providers to enhance accessibility and inclusivity in the community

What You'll Need:

To succeed in this role, you'll require:

  • A genuine desire to empower people with disability and promote independence
  • Relevant qualifications in disability, allied health, or community services, or lived experience with disability or caring for someone with disability
  • Demonstrated ability to meet and exceed performance targets
  • Excellent communication, listening, and relationship-building skills
  • Proven ability to work collaboratively with a wide range of stakeholders, including team members, individuals, and community service providers
  • Strong IT skills, with experience using CRM systems and Microsoft Office
  • A person-centered approach to outcomes, confidentiality, and professional integrity
  • Ability to travel extensively throughout your designated area

Salary: $70,000 - $80,000 per annum
