Community Support Worker

This position provides support and assistance to clients to undertake various activities and facilitate community engagement, meeting the client's need for social contact and/or company to participate in community life. Services may be provided within the home or while accessing community services. Activities may include, but are not limited to, food shopping, personal shopping, social activities, and accompanying health appointments. This role will support some clients with Personal Care, Domestic Assistance, and Respite care.
Key Responsibilities- Provide support and assistance to clients to undertake various activities and facilitate community engagement.
- Meet the client's need for social contact and/or company to participate in community life.
- Provide services within the home or while accessing community services.
- Support clients with Personal Care, Domestic Assistance, and Respite care.
- Certificate III Individual Support (formerly HACC III).
- FIRST AID/CPR (or willing to obtain).
- Current driver's licence.
- Safe and reliable car to undertake Social Support Individual duties.
- Extensive experience in working with challenging consumers and ability to relate to people with a variety of disabilities or age-related disorders.
- Demonstrated understanding of the needs and issues facing older people, people with disabilities, and their carers.
- Ability to report where there is a variation in a consumer's health, or Person-Centred Plan, or request for increased services.
- Ability to carry out ongoing safety checks on households and to report any unsafe situations in a consumer's home.
- Ability to establish a working relationship with each consumer and to contribute towards enhancing their self-esteem and independence.
- Functional Capacity Assessment.
- Working with Children Check.
- Police Check.
