We are seeking an experienced professional to join our team as a Case Manager in the Drug and Alcohol sector.

The successful candidate will be responsible for providing case management services to a group of identified complex consumers, focusing on establishing recovery goals and building personal strengths through skills training and education.

Key responsibilities include:

  • Utilizing therapeutic interventions to provide harm minimisation education
  • Facilitating continuity of care and improving equity of access to treatment by prioritising vulnerable and at-risk clients

Requirements include:

  • A good understanding of recovery-oriented care and relevant legislations
  • Ability to conduct assessments, treatment planning and goal setting

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a dynamic environment.
