We are seeking a highly skilled Clinical Care Coordinator to join our team at Open Minds Integrated Mental Health Services. As a Clinical Care Coordinator, you will play a critical role in delivering essential services across the Country to Coast Queensland (CCQ) region.
Key Responsibilities- Conduct thorough clinical assessments and develop tailored treatment plans for individuals with severe mental health conditions.
- Deliver evidence-based psychological interventions and manage a clinical caseload.
- Conduct risk assessments with detailed management plans and monitor and escalate mental health needs as necessary.
- Collaborate with multidisciplinary teams and external stakeholders to provide comprehensive care.
- Engage in data collection and contribute to monitoring and evaluation.
- Current annual practicing certificate from AHPRA.
- Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline.
- Open driver licence.
- Undergo a current or satisfactory Police Check.
- Working with children check (Blue card) or ability to obtain.
- Flexibility to travel and work after hours.
- Passionate about making a difference in the mental health sector.
- Experience with complex mental health presentations.
- Experience working in cross-cultural settings, particularly with First Nations people.
- Experience in trauma-informed and recovery-oriented services.
- Strong stakeholder engagement, organisational, and communication skills.
- Ability to manage multiple tasks, navigate challenging situations, and work within a team.
