Key Responsibilities
  • Provide high-quality Social Work services to patients, including assessment, planning, and intervention to address their psychosocial needs.
  • Collaborate with healthcare teams to develop and implement patient-centered care plans.
  • Contribute to the development and monitoring of the Social Work role in the context of Health.
  • Participate in quality improvement initiatives to enhance patient outcomes and experience.
  • Develop and maintain effective relationships with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Degree in Social Work or other equivalent qualification providing eligibility for membership of AASW.
  • Highly demonstrated expertise and knowledge of Social Work in the context of Health.
  • Demonstrated excellence in completing psychosocial assessments, planning, and intervention for complex patient/family presentations in the Health context.
  • Demonstrated skills to participate in the development/monitoring/implementation and review of the Social Work role in an identified setting.
  • Demonstrated commitment and highly developed skills in working with a team approach, alongside other disciplines and external agencies.
  • Demonstrated excellence in verbal, written, and computer skills, and ability to manage a significant workload.
