Job Title: Home Care Coordinator

This role plays a pivotal part in planning, coordinating, and monitoring care and support services for clients.

About the Job
  • Receiving referrals and conducting client assessments to tailor care plans
  • Developing individualised care plans aligned with client goals and needs
  • Coordinating staff schedules and matching carers with clients
  • Maintaining quality service delivery and ensuring client satisfaction
  • Keeping accurate client records and adhering to relevant policies and procedures
  • Responding promptly to incidents, feedback, or concerns
Requirements
  • A minimum Certificate III in Aged Care, Individual Support or equivalent is required (essential)
  • A Current First Aid Certificate is mandatory
  • A Valid Australian Driver's Licence and access to a reliable vehicle are necessary
  • A Current Police Check is required
  • An NDIS Worker Screening Check must be completed
  • Experience working with home care packages is advantageous
  • Strong interpersonal skills, enabling effective relationships with clients, families, and staff
  • A sound understanding of the Aged Care Quality Standards and person-centred care principles is essential
  • Proficiency in care management systems and Microsoft Office
  • Excellent time management and multitasking abilities

The hourly rate is $41.07, plus superannuation and salary packaging benefits.
