The role of a Hospital in the Home Registered Nurse is pivotal within our healthcare team. This position requires delivering high-quality, patient-centred nursing care to individuals in their own homes.
Key Responsibilities:
- Deliver expert nursing care to patients at home, utilising best practice evidence and collaboration with the multidisciplinary healthcare team.
- Supervise and support less experienced clinicians to achieve person-focused quality care.
- Develop and implement individualised care plans that meet the unique needs of each patient.
- Prioritise and organise workload effectively to deliver care with integrity and accountability.
Selection Criteria:
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- Demonstrated competencies and knowledge of nursing care in the context of a multidisciplinary team.
- Demonstrated effective clinical communication through information technology, written and verbal means, and sound interpersonal skills with the capacity to interact in a professional manner.
- Demonstrated application of evidence-based practice and critical thinking to advocate for and implement individualised person-centred care with a commitment to quality improvement.
- Demonstrated ability to supervise, delegate, and negotiate with others to deliver quality nursing care, with leadership and willingness to embrace change in the coordination and delivery of care.
- Recent clinical experience relevant to the position, including evidence of participation in professional development and maintenance of competence.
