Job Opportunity: Case Manager Role in Home Based Care

About the Job:

  • Provide assessment and support services to kinship carers and children, promoting stability, safety, and developmental needs.
  • Conduct comprehensive assessments for Kinship placements, working collaboratively with carers to achieve set goals.
  • Liaise with other professionals as needed, making referrals to ensure the best outcomes.

Responsibilities:

  1. Prepare thorough assessments for Kinship placements.
  2. Assess and identify strengths and vulnerabilities of placements.
  3. Work closely with carers to provide ongoing support and guidance.
  4. Communicate effectively with all stakeholders, including carers, children, and other professionals.
