Lead Clinical Care Professional

The role of Lead Clinical Care Professional at our Woodville care home is to provide exceptional person-centered care to residents. As a key member of the clinical team, you will work closely with General Managers to ensure the highest standard of ongoing assessment, care planning, and evaluation.
Your responsibilities will include overseeing daily clinical operations, supporting and leading the care home clinical team, reviewing clinical incidents, complaints, and changes, and ensuring compliance with relevant assessments.
You will also be responsible for managing person-centered care planning and evaluation for residents, including case conferencing with families and carers. Additionally, you will prepare and conduct clinical governance audits and oversee medication management in accordance with resident needs and legislative requirements.
To succeed in this role, you will need to possess excellent clinical assessment skills, care planning capabilities, and communication, leadership, and relationship management skills. You will also require an understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
We are seeking an experienced healthcare professional with a Bachelor of Nursing degree and current APHRA registration. Postgraduate qualifications in aged care are highly regarded, as are prior experiences in managing large care teams and delivering quality clinical care within an aged care or acute setting.
