About Us

Aged Care Resumes is a leading provider of aged care services in Western Queensland. Our team of dedicated professionals is passionate about supporting older people and individuals with disabilities to live their best lives.

The Community Connector plays a critical role in our organisation, providing specialist assistance to help older people understand and access aged care services. This involves utilising an assertive outreach approach to engage and build rapport with potential clients, as well as local intermediaries, stakeholders, and networks.

Key Responsibilities

  • Proactive outreach to identify and engage with potential clients and stakeholders.
  • Support clients to better understand and access aged care services, including My Aged Care.
  • Engage with community supports and stakeholders.
  • Conduct regular check-ins and follow-up with clients after services begin.
  • Address clients' psychological, emotional, and physical needs, and remove barriers.
  • Network with other organisations to promote ADA Link services.
  • Maintain high standards of documentation and consultation.
  • Develop and meet targets outlined in the annual work plan.
  • Liaise with other care finder services and support groups for older people.
  • Forge strategic partnerships with diverse cultural and community groups.

Requirements

  • Experience working with older people or disadvantaged/vulnerable groups.
  • Knowledge of operating within a social justice framework.
  • Social work or similar qualification (desirable).
  • Ability to understand and apply legislation, policies, and procedures.
  • Proven ability to problem-solve and find solutions for clients.
  • Excellent written and verbal communication and presentation skills.
  • High-level computer skills – Microsoft Office Suite or equivalent.
  • Commitment to delivering high-quality service.
