We are seeking a highly skilled and experienced Nurse Practitioner to join our team at the State Government of Victoria, Australia. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our healthcare services.
Key Responsibilities- Provide evidence-based, high-quality care to patients, including assessment, diagnosis, and treatment.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Contribute to the development and implementation of healthcare policies and procedures.
- Hold a Certificate IV or Diploma in Nursing with Medication Endorsement.
- Have excellent time management, communication, and interpersonal skills.
- Demonstrate sound clinical knowledge and excellent clinical skills.
- Provide person-centred care that meets best practice standards and aligns with our healthcare requirements.
- Experience in Aged care is desirable.
