Key Responsibilities- Provide health promotion and education to clients and carers, empowering them to take control of their health and wellbeing.
- Promote and deliver our award-winning Wellness for Independence programs, ensuring clients receive the best possible care and support.
- Monitor and promote health outcomes via Telehealth, leveraging technology to enhance patient care and engagement.
- Provide direct care services across a range of funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
- Assess, develop, and evaluate nursing care for all clients, utilizing a collaborative and person-centered approach.
- Work with clients to develop and achieve client care plans that are goal-driven and client-centered, promoting independence and autonomy.
- Assist clients to become independent with managing health needs through a Wellness and Reablement approach, working in conjunction with Allied Health clinicians.
- Understanding of community nursing and primary health care principles, including providing services in a consumer-directed care framework.
- Community, acute, and aged care experience of 1 or more years, with a strong focus on independent clinical practice or team-based work.
- Proficient in the use of electronic technology, including smartphones, tablets, laptops, email, internet, and client data management systems.
- Division One Registered Nurse with the Australian Health Practitioners' Regulation Authority (AHPRA).
- Experience providing advanced clinical care, including chronic disease management, medication management, palliative care, catheter management, stoma care, wound management, and diabetes management.
- Established holistic assessment skills and proficient practical skills.
- NDIS Worker Screening Check or willingness to obtain.
- Current driver's license and ability to travel for client care.
