Care Coordination Role in Richmond
In this part-time fixed term position, you will be responsible for ensuring seamless care pathways for patients with pancreatic cancer within our cancer services. You will be part of a multidisciplinary team and report to the Deputy Director.
Your Impact:
- Oversee patient journeys and coordinate care to ensure ease of access and smooth transitions between points of care.
- Provide leadership and clinical expertise by educating and guiding nursing teams and multidisciplinary teams to ensure streamlined patient journeys.
- Facilitate quality patient outcomes through efficient and cost-effective frameworks.
- Develop and implement quality activities promoting evidence-based practice.
To Be Successful in This Role
You will be a strong advocate with a compassionate approach. You will have highly developed interpersonal skills to engage with multidisciplinary teams and build relationships with patients, families, and internal/external stakeholders.
Your Qualifications:
- Cancer nursing experience - minimum 5 years clinical experience.
- Previous experience in cancer care coordination (desirable).
- Previous experience in palliative care nursing (desirable).
- Previous experience in clinical research/trials (desirable).
