About the Role

The Care Manager will lead with purpose - supporting their team and making a meaningful difference to the lives of residents every day.

This role is responsible for managing and leading the delivery of person-centred care to residents, ensuring the highest standard of ongoing assessment, care planning and evaluation, and clinical governance.

Main Responsibilities:

  • Oversee daily clinical operations of the care home to ensure residents receive the best care available.
  • Support and lead the care home clinical team, including coaching, mentoring, staff allocations and delegation, performance management, and education.
  • Review clinical incidents, complaints, and changes, providing solutions to reduce risks.
  • Lead compliance with AN-ACC assessments.
  • Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers.
  • Foster a strong team environment.
  • Prepare and conduct clinical governance audits.
  • Provide insights and innovative solutions for continuous clinical improvement.
  • Oversee medication management according to resident needs and legislative requirements.
  • Liaise with allied health, medical practitioners, and other services to provide optimal resident care.

Requirements:

  • Bachelor of Nursing with current APHRA registration is essential.
  • Postgraduate qualification in aged care highly regarded.
  • Strong leadership, communication, and interpersonal skills.
  • A person-centred care mindset.
  • A passion for nurturing a strong team environment.
  • Ingrained passion and empathy for residents.
  • Proven track record in delivering quality clinical care within an aged care or acute setting.
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
  • Proven experience in fostering a reporting culture balanced with effective investigative skills.
  • An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
