About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Avivo. As a Support Worker, you will play a vital role in enabling individuals with disability, ageing, or mental ill-health to live their best life in their own home and stay connected to their community.

As a Support Worker, you will provide individualized support services to our customers, including personal care, home management, and social care assistance. You will work in partnership with individuals and their families to develop and implement personalized support plans that promote independence, dignity, and quality of life.

Key Responsibilities
  • Provide personal care and support to individuals with disability, ageing, or mental ill-health
  • Assist with home management tasks, such as cleaning, laundry, and meal preparation
  • Support individuals to engage in social activities and maintain connections with their community
  • Administer medication (training provided)
  • Perform manual tasks, such as hoisting, to support individuals with physical disabilities
  • Travel between customers to provide support and services
Requirements
  • Reliability, flexibility, and a positive attitude
  • Excellent people skills and ability to build rapport with individuals and their families
  • Competency with using a smartphone and apps
  • Availability to attend mandatory and paid training during the week
  • A valid driver's licence and a reliable vehicle (essential)
  • Current NDIS Worker Screening check or willingness to obtain one
